Transitions Project

DSLC's Transitions Project helps individuals living in convalescent hospitals and skilled-nursing facilities to relocate into their own apartments and homes. We connect the individual to the services they require for independent living, affording them greater opportunities and quality of life.

The project was developed out of an increased need for institutionalized individuals of all ages with significicant disabilities to live independently.

Working in partnership with physicians, social workers, discharge planners, nursing homes and family members, Transitions Project staff and the client will develop a plan, set goals and explore resources in order to bring about a smooth transition into the community.
